The
Clinic Recorder is responsible for maintaining the Recorder’s sheet’s
and the Incident Log and completing the Clinic Report, as well as
documenting the vaccination status of each cat.
The Spay and Neuter Stations will administer two vaccines, one that
protects against panleukopenia/herpes virus/calicivirus/feline leukemia and one
that protects against rabies. Rabies
certificates originate from the Spay and Neuter stations and will be signed by
the surgeon or vet student administering the vaccines. Spay Assistants and Neuter vet students will
draw up the vaccines and keep the empty vials for the Recorder to collect. Clinic Recorder assures that all females
leaving the Spay Stations and all males leaving the Neuter Station are recorded
on the Recorder’s sheets. The Recorder’s
sheet is required by the State Veterinary Board. The Incident Logs
are reviewed by the Board of Directors to improve clinic procedures and ensure
clinic safety, and the Clinic Report is used to plan and improve future
clinics.
Before the Clinic
1)
OBTAIN
the Recorder’s Notebook from the Supply Coordinator. This includes the Recorder’s sheets, incident
log, clinic report, color chart, list of spay surgeons, list of vet students
During the Clinic
1)
LIST the names of all
surgeons, spay vet students, and neuter vet students. Update list as needed throughout the clinic.
2)
STAMP or write the
organization name, address and phone number on all three copies of the rabies
certificate. This may be done before
surgery begins by collecting and stamping the certificate books.

3)
COMPLETE or correct
the rabies certificate if necessary, filling the following fields:
Ø SPECIES (Cat)
Ø SEX & “Altered”
Ø AGE
Ø SIZE (Under 20 pounds)
Ø PREDOMINANT BREED (DSH, DLH, Siamese, etc.)
Ø COLORS
Ø DATE VACCINATED (Clinic date)
Ø VACCINATION EXPIRES (1 yr. from clinic date, not
3 years—as this is probably the cat’s first vaccination and a booster shot is
not likely)
Ø PRODUCER (First three letters of the rabies vaccine
manufacturer and lot number found on the vial)
Ø NAME (Use the F#.)
Check off the following vaccines:
Ø RABIES
Ø FELINE LEUKEMIA
Ø FELINE PANLEUKOPENIA
Ø FELINE RHINOTRACHEITIS
Ø FELINE CALICIVIRUS
4)
VERIFY the veterinarian’s
signature. Advise the Spay or Neuter Transporter if you need a signature.
5)
PEEL off the vaccine
labels from the vials and attach them to the white copy of the certificates.
Also apply a Revolution sticker if Revolution will be applied.
6)
RECORD from the Medical
Summary Slips the following information:
Ø Gender and color of cat
Ø Cat’s pre-operative status: Pregnant, Lactating,
In Heat, Kitten, Cryptorchid (See the Medical Summary
Slips)
Ø Number of fetuses (if pregnant)
Ø Surgeon or vet student who performed the surgery
Ø Spay site: Flank or Midline, double check by
look at the cat
Ø Fluids or other post-surgery treatment ordered
Ø Other observations or complications
NO CAT (MALE OR FEMALE – ALIVE OR DEAD) MAY LEAVE THE SURGERY
AREA
WITHOUT BEING RECORDED ON THE Recorder’s Sheet.
7)
FILE the medical
summary slips and rabies certificates in numerical order in the file box.
8)
MEET with the
Discharge Supervisor prior to discharge to explain any relevant medical
problems that must be communicated to the caretakers.
9)
GIVE the file box to
the Discharge Supervisor.
10)
MAINTAIN the Incident
Log, a record of all problems (no matter how trivial) that occur during the
clinic.
After the Clinic
1)
PREPARE the Clinic
Report, including:
Ø Number of veterinarians, volunteers and cats
Ø Volunteer start time and end time
Ø Surgery start time and end time
Ø Hours of surgery
Ø Number of cats per vet per hour
Ø Discharge end time
2)
SUBMIT all logs and
reports to the Clinic Supervisor or Supply Coordinator.

Transporter Instructions
The Recorder/Rabies Certificate Station Transporters are
responsible for delivering cats to the Medications Station; monitoring each
cat; collecting vaccine vials and returning spay boards to the Spay Board
Station.
1)
COLLECT the empty vaccine vials from
the Spay Station supply table throughout the clinic.
2)
UNTIE secured cats from the spay
boards.
3)
VERIFY that the F# of the paw tag
matches the F# on the rabies certificate and Medical Summary Slip. If
they do not match, notify the Spay or Neuter Transporter who delivered the cat
that the paperwork is for a different cat.
4)
MONITOR the cats carefully. Respiration (>6/minute), heartbeat
(>100/minute), and gum color (pink).
If the cat is not breathing well or its gums are blue, pull firmly on
the tongue, press a fingernail on the nose leather, and NOTIFY THE CLINIC
SUPERVISOR immediately for
assessment.
5)
TRANSPORT cats to the Medication
Station once both the Certificate Recorder and Clinic Recorder have completed
the documentation.
6)
NOTIFY the Medications Station if any
cat is flagged as contagious. Change your gloves and clean the table after
handling any URI or otherwise infectious cats.
7)
RETURN spay boards to the Spay Board
Station. Take care not to mix the soiled boards with the clean boards.
8)
CLEAN-UP sweep/vacuum
your area; wash tables; bag all trash and take to the dumpster; mop the area
until no cat odor remains.
9)
REPACK the station kit, inventory
supplies (see Supply List in the kit).
10)
RETURN the kit and any extra items to the
Supply Coordinator.
